The State of Care at Home: Trends, Challenges and Opportunities for Evolving Care Models

Care-at-home programs present one opportunity to address many of the ongoing challenges health systems are facing, while also improving experiences for patients and providers. But safely relocating care from traditional brick-and-mortar facilities to a patient’s home is no easy task.

The survey focused on the opportunities and needs driving care at home, the value leaders see from these programs, and the challenges to scaling care-at-home experiences.
