“For one patient, after several weeks of continuous monitoring I felt like we had to pry [the device] off of her. She just loved the comfort and the security of knowing that somebody was keeping an eye on her 24/7.”

Transitional Care Pathway Day One

Enroll patients from a clinic, ED, or after a telehealth encounter.
Monday
9:00 am

Cynthia, a 65-year-old patient with COPD, experienced a moderate exacerbation during winter weather.

Her provider enrolls her in the Current Health platform for a 14-day care period.

Transitional Care Pathway Day One

Our 24/7 technical support ensures successful setup.
Monday
2:00 pm

Cynthia receives a Current Health kit at her doorstep. The kit is custom configured to her clinical situation, including the Current Health wearable and a spirometer.

It takes Cynthia just a few minutes to link the spirometer to the Current Health app on her phone. She receives a call from our technical support team to answer any questions.

Transitional Care Pathway Day Two

Customize pathway prompts for a population or individual.
Tuesday
9:00 am

Cynthia starts her day with a prompt to submit a spirometry reading via the MIR Spirometer and to complete a brief symptom assessment via the Current Health app.

Her physician can see Cynthia’s readings and self-reported symptoms through the dashboard.

Transitional Care Pathway Day Three

Use our in-home service partners as an extension of your team.
Wednesday
3:00 pm

WorkPath arrives at Cynthia’s house to collect her sputum sample.

A little while later, ScriptDrop delivers her adjusted medication.

Transitional Care Pathway Day Three

Our recommended alarm thresholds are based on extensive data collection.
Wednesday
1:00 pm

Cynthia develops a worsening cough and shortness of breath. The Current Health continuous monitoring device detects her decreased oxygen saturation and triggers an alarm.

Her care team messages her to check in. Her provider orders a sputum sample collection, a steroid, and an antibiotic, all through the Current Health platform.

Transitional Care Pathway Day Four

Patients can connect with their care team at any time.
Thursday
10:00 am

Cynthia continues completing her daily spirometry readings and symptom reports. She has a call with her physician every few days until her symptoms are under control.

Transitional Care Pathway Day Fifteen

End-to-end logistics management makes discharge a breeze.
Monday
11:00 am

After two weeks, Cynthia’s COPD is back under control.

A Current Health logistics partner collects the kit from her front door.
